Healthwise Launches its FHIR® App in Epic’s App Orchard, Improving Care Coordination with Evidence-Based Patient Education
BOISE, Idaho, Sept. 24, 2018 (GLOBE NEWSWIRE) -- Healthwise announced today the availability of its second patient education app in Epic’s App Orchard. The Healthwise for Care Coordination app makes it easier to integrate patient education into the Epic workflow and reach patients beyond the four walls of the hospital to help improve patient experiences and outcomes. Organizations can support both point of care and care coordination interactions with a single source of patient education using the point-of-care app (also listed in the App Orchard) and the new care coordination app.
“The new app enables care teams to provide patients with current, relevant, and dynamic health information, see how people are interacting with the education, and better respond to patients’ needs,” said Dr. Adam Husney, President and Chief Medical Officer, Healthwise. “This is an exciting new way to use technology to help care teams work more efficiently and create the personalized experiences that patients expect.”
The Healthwise for Care Coordination solution, paired with FHIR technology, helps care coordinators easily integrate patient education into their Epic care coordination workflow. The app and supporting APIs are cloud-hosted, and the content updates automatically, so clinicians and patients always have the most up-to-date health information.
Care coordinators can select from Healthwise’s award-winning resources, including engaging videos and shared decision making tools. Education can be printed, sent to a secure messaging center, or delivered to the patient’s MyChart portal™ so that the information is available anytime, anywhere, on any device. The entire care team can see what information patients received either at the point of care or at home.
