LifeMasters Contracts with Prevea Health Plan to Provide Disease Management Services
IRVINE, Calif., July 13 /PRNewswire/ -- LifeMasters Supported SelfCare, Inc. a leading disease management company, today announced it has signed an agreement to provide disease management services to Prevea Health Plan (PHP) members with diabetes, asthma, and coronary artery disease (CAD). Prevea Health Plan offers customizable health insurance plans to northeastern Wisconsin employers in six county service areas including Brown, Door, Oconto, Kewaunee, Manitowoc and Outagamie counties.
Under the three year contract, members enrolled in Prevea Health Plan's commercial HMO and POS plans will have access to disease management services, including customized health education, lifestyle-modification support, regular telephonic interaction with a clinical nurse consultant and regular monitoring of the member's vital signs and symptoms for early identification of changes in health status. In addition, LifeMasters communicates with the member's physician whenever a clinical problem arises.
"LifeMasters is excited to work with Prevea Health Plan to provide disease management services to their members," said Christobel Selecky, LifeMasters Chairman and CEO. "Our disease management program ensures that each participant receives the education and coaching necessary so that there are no gaps in their health care, while helping to manage the costs incurred by health plans. This approach ensures that all players in the delivery of health care -- the member, physician and health plan -- benefit from increased communication and care that a disease management program provides."
"Promoting health and providing the highest quality of healthcare is part of our mission," said Thomas Huffer, M.D., Medical Director of Quality for Prevea Health Plan. "We chose LifeMasters because of their reputation for quality and feel that their program will help us promote the health of our patients with chronic illnesses."
Studies have shown that medical expenses incurred by individuals with chronic illness account for a disproportionate share of health care expenditures in this country. By keeping individuals healthy and following the best standards of care, preventable hospital admissions, hospital lengths of stay, and emergency department visits are reduced along with the associated costs.
